1. Regional meetings of Part C and Section 619 coordinators including presentations of effective transition practices and partnerships

2. Establishment of a Community of Practice focusing on the issue of transition

3. Delineation of resources and supports to improve transition from Part C to Part B/619 services

4. Assistance in aligning monitoring systems to ensure the ability of states to report on the correction of noncompliance and target the data needed to address this indicator
