• Improvement Activities:

• A new automation system that will have the capacity to make the transition process seamless by giving access to both Part B and Part C child records

• Annual Child count data will be reviewed and data verification reports run to improve the timeliness and accuracy of exit data

• Monitor the effectiveness of improvement strategies related to transition by review of statewide and disaggregated compliance data, data on exiting children, family survey results, and issues identified in complaints
